Application Forms and Key Dates

At the bottom of this page you may access the APPLICATION FORM for Pilgrims and Peer Support (PS).

You may download them (Microsoft Word format) and fill them in electronically. Please make sure you include the photograph where required and sign the documents where specified.

Application Forms for POH; India, Nepal, Fiji, Alice Springs and Bali must be submitted directly to your School based POH Coordinator.
Application Forms for the MC Soup Kitchen in Perth must be posted direct to Brother James. Please note that note that the Application Form is 'universal' and as such, some of the questions are not applicable to the MC Soupt Kicthen in Perth
PSS Application Forms should be posted to Brother James. Additional instructions can be be found on the forms.
